Here is another source, this one is made by the Nucleonic Company of America and probably went with an Ore-Lokator or similar instrument. It probably goes in the holder on the front of the instrument seen here: http://www.orau.org/ptp/collection/surveymeters/NCARM2.htm
The picture is horrible and of the envelop it came in. This is because it is mildly leaking, but no worries, it does not contain much activity(good thing for rad-decon spray)!

My dad has very kindly built me display shelves at his place of business. The room is narrow so it is difficult to take good pictures. Only items that are not or very low level radioactive are here, but it is a good environment for them.
